Pay Rate: $45 per visit, plus mileage
RESPONSIBILITIES:
- Complete initial and supervisory assessments to identify the needs of our members
- Vital sign documentation of Member if appropriate at time of visit
- PRN Registered Nurse visits will be conducted and documented every 62 calendar days after initial assessment or more often if the Medical Assistance Division (MAP) eligible recipient’s condition warrants it
- Responsible for conducting and documenting visits at the MAP eligible recipient’s residence for the purpose of assessing his/her progress and PCS attendant’s performance
- Assist in developing and guiding the Individual Treatment Plan (ITP) and updating as indicated and in cooperation with the MAP eligible recipients Managed Care Organization (MCO) Care Coordinator
- Complete electronic/paper compliant documentation of the care provided in the system of record
- Coordination of care
- Deliver New Employee Orientation (NEO) and New Member Orientation (NMO) paperwork for completion by Member and Personal Care Attendant, this includes identification of Legally Responsible Individual (LRI) and completion of LRI documentation
- Communicate with family members, physicians, care coordinators, and other prescribed individuals regarding the health of the members
- Provide effective communication to patients/clients, their family members, team members, and other health care professionals
- Designated services areas include; Bernalillo, Sandoval, Santa Fe, Socorro, Torrance and Valencia Counties.
WHAT YOU BRING TO THE TABLE:
- Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, with a minimum of one-year direct patient care experience
- Current New Mexico state license as an RN
- Proof of current CPR certification
- Compassionate, friendly demeanor
- Excellent communication skills
- Negative TB test
- Maintain required insurance
- Good interpersonal skills
- Hand-eye coordination and manual dexterity
- Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients
- Ability to operate under stressful conditions and make quick decisions
- Must have dependable transportation and be willing to commute to the Member’s home
- Ability to provide basic medical management
