Position Summary:
The Director of Population Health provides strategic, clinical, and operational leadership for St. Luke’s population health programs. This includes oversight of value-based care strategy, care coordination, quality performance, population health analytics, regulatory compliance, and interdisciplinary partnerships. The role also supports St. Luke’s Medical Group quality and patient experience teams. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke’s mission and values.
Education, Experience, & Licensing Requirements:
Education: Bachelor’s in healthcare field required. Master’s degree in Healthcare Administration, Public Health, Business Administration, Nursing, or related field (MPH, MHA, MBA, or MSN preferred)
Experience: 5+ years of progressive leadership experience in population health, value-based care, care coordination, or clinical transformation in a health system, ACO, or payer setting; Demonstrated success in leading multidisciplinary teams and managing large-scale population health programs. Experience with HEDIS, MIPS, STAR ratings, CMS quality programs, and risk-based contracting.
