Telephonic Medical Assistant

Ennoble Care is a mobile primary, palliative, and hospice care provider serving multiple states. They seek a Telephonic Medical Assistant in Hackensack, NJ to support care coordination and patient communication, ensuring high-quality care delivery.

Responsibilities

  • Complete individualized patient care plans and perform care management using electronic medical records
  • Maintain frequent contact with patients to support care coordination and improve compliance
  • Accurately document all client communications
  • Communicate emergent needs between patients and clinicians
  • Keep supervisor informed of issues related to care plans
  • Collaborate across organizational areas to meet patient and family needs
  • Use technology such as secure email, telephone systems, and electronic medical records effectively
  • Follow documentation protocols and escalate client issues appropriately
  • Provide excellent customer service and support team members productively
  • Operate office equipment like computers, telephones, and copy machines

Education

  • High school diploma or equivalent preferred
