The Licensed Social Worker works with a multidisciplinary treatment team to provide psychological, social, and interventions that support patients and families relating to behavioral/medical health, levels of care, disease and illness and wellness and recovery.
This position is home-based in Ocean and Monmouth counties, with additional potential for coverage and other settings. You will see patients in Tinton Falls and in their homes throughout these counties. Hospice/palliative experience is strongly preferred.
