Hospice Intake RN
Responsibilities
Coordinates the referral intake process under the direction of the Clinical Supervisor/Manager/Director. Records and maintains timely and accurate referral data. Provides clinicians with required information needed to start services for a new patient, and contacts physician for any concerns.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
1. Previous home care experience preferred.
2. Graduate from an accredited school of nursing.
3. Current license as a registered nurse.
4. Familiar with state and regulatory requirements.
5. Strong verbal and written communication skills. Strong interpersonal skills. Strong problem solving skills. Strong time management skills. Ability to work independently.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $66,560 to $92,934.40 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
