Responsibilities
- Manage diverse patient caseload ensuring quality care and coordination
- Provide coaching and oversight to CNAs to maintain high clinical standards
- Develop and execute individualized care plans based on assessments
- Coordinate with PCPs, therapy providers, and internal teams for comprehensive care
- Educate and support patients and families on healthcare needs and treatments
- Maintain accurate, timely clinical documentation adhering to regulatory standards
- Communicate effectively with care team and escalate clinical issues appropriately
- Participate in on-call rotations including weekends, holidays, and possible night coverage
- Monitor patient readmission rates and implement quality improvement initiatives
- Ensure administrative tasks such as mileage reimbursements are completed properly
Education
- Associate’s Degree or Bachelor Degree in Nursing
- Valid Registered Nurse license in Colorado or compact multi-state license
