Home Health RN Case Manager

Abby Care is a pioneering home health provider dedicated to transforming care for vulnerable families using innovative technology and a unique care model that centers on clinical excellence and family support. They are seeking a compassionate, organized RN Case Manager to oversee comprehensive patient care and coordinate services to achieve optimal outcomes.

Skills

  • Excellent communication and organizational skills
  • Strong clinical judgment and patient assessment abilities
  • Proficiency in electronic clinical documentation and real-time charting
  • Ability to collaborate across interdisciplinary care teams
  • Knowledge of Medicaid paid family caregiving programs and CNA scope preferred
  • Familiarity with OASIS quality measures preferred

Responsibilities

  • Manage diverse patient caseload ensuring quality care and coordination
  • Provide coaching and oversight to CNAs to maintain high clinical standards
  • Develop and execute individualized care plans based on assessments
  • Coordinate with PCPs, therapy providers, and internal teams for comprehensive care
  • Educate and support patients and families on healthcare needs and treatments
  • Maintain accurate, timely clinical documentation adhering to regulatory standards
  • Communicate effectively with care team and escalate clinical issues appropriately
  • Participate in on-call rotations including weekends, holidays, and possible night coverage
  • Monitor patient readmission rates and implement quality improvement initiatives
  • Ensure administrative tasks such as mileage reimbursements are completed properly

Education

  • Associate’s Degree or Bachelor Degree in Nursing
  • Valid Registered Nurse license in Colorado or compact multi-state license
