POSITION TITLE: Case Manager
PROGRAM/LOCATION: ADULT FULL SERVICE PARTNERSHIP (FSP) PROGRAM – SAN LUIS OBISPO
POSITION SUMMARY
The Full Service Partnership (FSP) Program provides intensive, community-based support to adults living with serious mental illness who are at risk of acute hospitalization or long-term institutional care. The primary objective of the program is to help clients achieve and maintain stability in the community through structured, individualized support. FSP serves individuals facing complex challenges, including co-occurring substance use disorders, significant trauma histories, and substantial functional impairments. Services are provided in the community—where clients live and feel most comfortable—rather than in clinical or hospital settings. This approach emphasizes hope, resiliency, and recovery, and is grounded in principles of cultural humility, professional integrity, and a commitment to ongoing self-reflection and personal growth for both clients and staff. The Case Manager plays a vital role in delivering trauma-informed, wraparound services tailored to each client’s unique needs. Responsibilities include individualized service planning, case management, co-occurring substance use support, medication coordination, psychosocial rehabilitation, linkage to housing and vocational services, and access to care on a 24/7 basis. Working in close collaboration with the TMHA Clinician, the Case Manager jointly supports a caseload of 15–20 adults. Core duties include crisis response, skill-building, transportation to appointments and essential errands, and ongoing support in daily living activities. The Case Manager ensures that each client receives the care and guidance necessary to foster independence and maintain wellness in the least restrictive environment.
HOURS/ SALARY
Full-time, non-exempt position. 40 hours per week. Anticipated hiring range is $24.50-$25.50. Includes a generous individual benefits package that includes health, dental, vision, life, 403b retirement (with up to a 5% employer match) paid vacation/holiday time, paid sick time and a variety of helpful employee perks as well. Responsibilities and Duties:
1. Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; along with the assigned clinician and treatment team, assume responsibility for developing, , implementing, evaluating and revising treatment goals and plans; adhering to all required reporting requirements; provide one-on-one support to clients as needed; provide symptom management support; educate and support clients’ families while acting in clients’ best interest and advocating for the rights of the clients. Assist clients in all aspects of independent living including the development of social, vocational, and effective life management skills; assist clients in the areas of proper nutrition, cooking, shopping, budgeting, housecleaning, and maintenance, and personal hygiene.
2. Assist the clinician in conducting a comprehensive assessment of psychiatric history, mental status, and diagnosis.
3. Provide ongoing assessment of clients’ psychiatric symptoms and their responses to treatment. Recognize and be available to clients in psychiatric crisis or emotional distress, and assist clients in developing appropriate coping and conflict resolution skills. Provide emotional support and symptom recognition and management. Educate clients and their families about the nature of mental illness, symptoms of mental illness, coping strategies, and medication. Assist clients in developing ways to manage symptoms, augment coping skills, and strategize problem-solving skills.
4. Assist clients in learning more about substance use and treatment options open to them such as A.A. and other support groups. Work with clients in identifying possible precursors that could lead to alcohol or drug abuse.
5. Facilitate client meetings, and assume the role of advocate in all aspects of clients’ needs and goals. This duty may include familiarizing clients with and encouraging the usage of community resources, support systems, services, and opportunities. Support clients in building healthy peer and familial support networks. Assist clients in developing various recreational skills and interests.
6. Discuss with clients what goals they plan to achieve for themselves and assist clients in determining, developing, and carrying out their own goals; review plans and goals, evaluate progress, and strengthen and support independence.
7. Provide ongoing assessment of activities of daily living. Assist clients in developing skills in the following areas, as needed, in order to achieve the highest level of independence possible: personal hygiene, shopping and cooking skills, money management skills, accessing reliable transportation, and scheduling and attending appointments.
8. Provide opportunities to allow clients to establish vocational goals and identify any job-related areas that they want to work on. Provide vocational counseling, referral to appropriate vocational services, and assist clients with job coaching and support as needed.
9. Encourages the cleanliness and upkeep of client residences; this duty may include general household chores, yard work, completion and/or reporting of needed repairs, acquisition and disposal of furniture, and teaching and assisting clients in maintaining a clean and sanitary living environment.
10. Complete program paperwork such as individual client files, evaluating and charting client progress, service plans, admission/discharge paperwork, Medi-Cal billing, client security deposit receipts, time sheets, petty cash, , and/or inventory forms.
11. MediCal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Team Leader and Director.
12. All direct service staff are expected to spend 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.
13. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA’s Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
14. Attend regular staff meetings, participate in client intake meetings, and complete required hours of in-service training
15. Assume other duties and responsibilities as assigned by the Clinical Team Leader.
Job Environment:
1. Daily and frequent documentation of client services on the computer.
2. Services may be delivered to clients during a Monday through Sunday workweek; schedules will be developed in collaboration with all teammates and management staff to ensure maximum support for clients.
3. Moderate, physical activity and frequent driving that will include the transportation of TMHA clients; may include moving or lifting up to 50 lbs.
4. Involves moderate risks that require safety precautions and the risk of exposure to potentially intense situations.
5. Position involves working within the residential environments.
6. Possible intense situations related to confrontational or oppositional adults.
Requirements
Minimum Requirements:
1. BA/BS AND 6 months of full-time experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time experience with target population, OR 4 years of full-time experience with target population.
2. Strong interpersonal skills, collaborative work style and strong oral/written and communication skills.
3. Well defined organizational and time management skills.
4. Ability to work independently and flexibly.

15. Conditions of employment include:
• Must be at least 18 years of age; possess a valid CA Driver’s License, at least 2 years driving experience and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize reliable personal vehicle and auto insurance.
• Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting.
• Must be able to successfully complete First Aid and CPR training.
